Key Responsibilities:

• Deliver personal care with dignity and respect, supporting individuals with daily living activities

• Promote independence by encouraging skills development in areas such as cooking, cleaning, and budgeting

• Support person-centred planning, working collaboratively with individuals, families, and professionals to achieve meaningful outcomes

• Assist with medication management, ensuring safe administration and monitoring

• Accompany individuals on outings, appointments, and social activities to promote inclusion and wellbeing

• Maintain a safe and secure environment, adhering to health and safety policies

• Communicate effectively, using a range of approaches bespoke to individual needs, including PBS strategies

• Record and report accurately, ensuring all incidents, concerns, and progress are documented in line with policy

• Participate in team meetings, contributing to shared learning and continuous improvement

• Provide emotional support, helping individuals build relationships and maintain a sense of belonging.

What We’re Looking For:

• Practitioners with empathy and a strong understanding of learning disabilities and autism

• Excellent communication and interpersonal skills

• Ability to work both independently and as part of a team

• Flexibility, adaptability, and a proactive approach

• Commitment to person-centred care and PBS principles

• Knowledge of health and safety procedures

• Confidence in recording and reporting accurately

• Experience in working within the framework of Positive Behaviour Support

• Understanding of the Mental Capacity Act and Deprivation of Liberty Safeguardings in the community

• Level 2 or 3 in Health and Social Care (or a willingness to work towards)

• Level 3 in Positive Behaviour Support (desirable)

• Practitioners with a driving licence

• Care Certificate
