Job Description:

Key Responsibilities:
-
Deliver personal care with dignity and respect, supporting individuals with daily living activities
-
Promote independence by encouraging skills development in areas such as cooking, cleaning, and budgeting
-
Support person-centred planning, working collaboratively with individuals, families, and professionals to achieve meaningful outcomes

Assist with medication management, ensuring safe administration and monitoring
-
Accompany individuals on outings, appointments, and social activities to promote inclusion and wellbeing
-
Maintain a safe and secure environment, adhering to health and safety policies
-
Communicate effectively, using a range of approaches bespoke to individual needs, including PBS strategies

Record and report accurately, ensuring all incidents, concerns, and progress are documented in line with policy
-
Participate in team meetings, contributing to shared learning and continuous improvement
-
Provide emotional support, helping individuals build relationships and maintain a sense of belonging.

What We’re Looking For:

Practitioners with empathy and a strong understanding of learning disabilities and autism
-
Excellent communication and interpersonal skills
-
Ability to work both independently and as part of a team
-
Flexibility, adaptability, and a proactive approach
-
Commitment to person-centred care and PBS principles
-
Knowledge of health and safety procedures
-
Confidence in recording and reporting accurately
-
Experience in working within the framework of Positive Behaviour Support
-
Understanding of the Mental Capacity Act and Deprivation of Liberty Safeguardings in the community
-
Level 2 or 3 in Health and Social Care (or a willingness to work towards)
-
Level 3 in Positive Behaviour Support (desirable)

Practitioners with a driving licence

Care Certificate
